New Patients
Our office is committed to providing you with the best dental experience and the best dental care. We can help achieve this with a little preparation by you before your appointment.
1. Prepare a list of any dental or insurance related questions you may have for our office.
2. If you have dental insurance, please bring your insurance card and subscribers ID number or social security number with you for claims processing. So that you are aware of any policy limitations, we ask that you thoroughly read your policy in advance. There are many types of policies out there. Some policies may base their reimbursement on an internal fee schedule which may or may not coincide with our fees. We accept all insurance policies that allow you to pick your own dentist. If you have a PPO policy, you can still choose our office for treatment. If you are not sure of the difference in reimbursement between a contracted PPO office and ours, feel free to ask our office to call your insurance and we can find out the information for you. Our office is also a Delta Premier participating provider. We do not participate in any DMO plans.
3. Please print out the patient information and medical history form from the link below. Complete it and bring it to your appointment. Also, please download and read the Privacy Practices Form and the Facts about Fillings Booklet.

Once you have arrived, our staff will be there to greet you and answer all questions you have regarding patient forms, scheduling, insurance and payment options (credit cards, debit cards, CareCredit, and checks).
We accept
We believe your time is very important and make every effort to seat you as close to your appointment time as possible. Once you have been seated in the treatment room, it will be determined whether or not x-rays are needed. If so, we will take the necessary digital x-rays (1/3 the radiation of conventional x-rays). Dr. Dispenziere will then spend some time addressing any questions or concerns you may have before treatment options are discussed. Each of our dental hygienists have extensive knowledge from their time spent working in periodontal offices. They are here to provide you with the best care in our office as well as to instruct you on preventive at-home care for your teeth and gums.
